    How have the derived the 2 nd and 3rd condition in part 2 of Ques 5?

    Redington's theory of Immunisation, and the associated formulae (to which you refer) were in the ST5 Core Reading in 2009. They are not in the SP5 Core Reading, and so you are not expected to be able to quote them.
